Enroll in FasTrak® to Avoid Paying a Toll Evasion Penalty   If you are a first-time toll violator, you can have your penalty dismissed by opening a FasTrak® account. Please follow the instructions below. Download a FasTrak® application form.   Complete and sign the FasTrak® application form.   Mail the following to the FasTrak® Customer Service Center: Completed and signed FasTrak® application form. Appropriate amount required to open a new FasTrak® account (e.g., if opening an account by check, the appropriate amount required would be $70 for one toll tag ($50 for prepaid tolls + $20 for toll tag deposit)).
