As an account holder, you should keep a copy of the Terms and Conditions for your records.
As an alternative to enrolling online, you can enroll by completing this application form and submitting it to the FasTrak Customer Service Center either by mail, fax, or in person.
As FasTrak customer, you will be responsible for maintaining your account. Before using FasTrak, new customers should read the Customer Handbook to become familiar with FasTrak, its account policies and toll violation information.
The Account Update Form is used by existing account holders to provide updated information (mailing address, phone numbers, or vehicle information) as well as to change your replenishment method.
The Account Closure Form is used to request closure of your FasTrak account. It should be filled out completely, signed, and submitted to the FasTrak Customer Service Center along with your toll tag(s). If no toll tag is returned, your deposit will be forfeited.
The Additional Toll Tag Request Form is used if you would like to add a toll tag to your existing FasTrak account. Additional refundable toll tag deposits may be required.
The Replacement Toll Tag Request Form is used if you are requesting a toll tag to replace one that you already have, which may be malfunctioning due to battery failure or is lost, stolen, or damaged.
The DMV registration dispute form is used to formally request a review of your outstanding violations, which are either already on DMV hold or may soon be sent for DMV hold. A request for review does not release the DMV hold on your registration.
